The Franciscan House Adult Day Health Care Program (ADHC) is designed as a healthcare day program for primarily older adults. The program is open Monday - Friday from 7:30 am to 5 pm and is staffed by an interdisciplinary team of nurses, social workers and care assistants. Daily programming includes activities of daily living, reality orientation, exercise, health and nutrition counseling and consultation, social and recreational activities, leisure skill development and educational opportunities. Each client is assessed for an individualized plan of care. The ultimate goal of all services is greater independence and community involvement to enable prevention of 24-hour residential care. While many clients attend five days per week, many attend three days per week for approximately five hours each day. A noon meal is provided, as are morning and afternoon snacks.
The program is licensed and certified for the Medicaid Waiver Program and Private Pay.
