The Franciscan House Adult Day Health Care Program (ADHC) is designed as a healthcare day program for primarily older adults. Our hours are 8 a.m. - 4:30 p.m., Monday - Friday. Our interdisciplinary team consists of a director, program manager, nurse, social worker (or designee), and direct service worker.
Daily programming includes activities of daily living, reality orientation, exercise, health and nutrition counseling and consultation, social and recreational activities, leisure skill development and educational opportunities. Each client is assessed for an individualized plan of care. The ultimate goal of all services is greater independence and community involvement to enable prevention of 24-hour residential care. Clients attend a minimum of five hours a day and receive a noon meal as well as morning and afternoon snacks.
The program is licensed and certified for the Medicaid Waiver Program and Private Pay.
